   we have adapted the toolkit and have various monitoring audit tools that can evidence compliance to the overall tool kit requirements .  The tool kit itself whilst valuable is very labour intensive to review in all clinical areas with any regularity.

   I read somewhere a few weeks ago that these had been adapted for community settings but can't find them anywhere. Has anyone seen them?
